They count calories, they exercise intensely and they restrict entire food groups. The weight returns every single time because the root cause remains untouched.

Your body possesses remarkable intelligence. It knows exactly how to maintain healthy weight when given the right conditions. THE WEIGHT LOSS MEDICATION BOOM: WHAT I WANT YOU TO UNDERSTAND

The explosion of GLP-1 medications,  Ozempic, Wegovy, Mounjaro, Zepbound, represents one of the most significant shifts in weight loss treatment in decades. In my clinical practice, I ask a different question: What happens when you stop taking these medications? The research provides a clear answer,  and it’s not encouraging.

What These Medications Actually Do

GLP-1 drugs mimic a hormone your body already produces naturally after eating. They slow down how quickly food leaves your stomach. They suppress appetite signals to your brain and they manipulate your blood sugar regulation.

The result? You feel full faster, so you eat less. You do lose weight,  often 15-20% of your body weight over a year.

Sounds promising, but here is what deserves your attention.

The Concerns Worth Understanding

You are signing up for life. The research makes this absolutely clear. The STEP 1 trial extension, published in Diabetes, Obesity and Metabolism in 2022, found that participants regained two-thirds of their prior weight loss within one year of stopping semaglutide. [5] A 2025 meta-analysis in eClinicalMedicine confirmed this pattern, finding that approximately 50-60% of lost weight is regained within one year after stopping GLP-1 medications. [6] This isn’t a short-term treatment. This represents a potential lifetime commitment to medication.

The cost adds up. List prices for GLP-1 medications range from approximately $1,000 to $1,350 per month without insurance. [7] Even with recent price reductions bringing cash prices to around $350 per month through direct-to-consumer channels, you’re looking at over $4,000 annually,  indefinitely. [8]

Gastrointestinal effects are common. Nausea, vomiting, diarrhea, and constipation affect a significant percentage of users. A pooled analysis of the STEP 1-3 trials found that 44% of participants on semaglutide experienced nausea, 30% experienced diarrhea, and 25% experienced vomiting. You are losing muscle, not just fat. This concerns me greatly. According to the American Diabetes Association, lean body mass can account for up to 15-40% of total weight loss from GLP-1 therapies. Long-term data remains limited. These medications have been used for weight loss at higher doses for only a few years. While diabetes formulations have longer track records, questions persist about effects we haven’t yet discovered with extended use. The FDA requires boxed warnings about thyroid cancer risk based on animal studies. [12]

The root cause remains completely untouched. This troubles me most. These drugs override your body’s signals and they don’t restore balance.
